Our services are available to anyone with sport- or exercise-related injuries or inquiries. To make an appointment, please contact the clinic receptionist at 416.978.4678, press “0”, or stay on the line for further information.
 
In order to gain access to the clinic you must swipe your valid student card, present an appointment card, or leave a piece of ID (not your health card or SIN card) at the turnstiles in the main lobby of the Athletic Centre. 
 
All new patients are required to complete a Registration Form and Health History Form.  In order to speed up the registration process you can download these forms and complete them prior to coming to your first visit.  Returning patients must update any information on their record to ensure it is current.  
 
All information gathered for treatment is confidential except as required or allowed by law or to facilitate diagnosis (assessment) or treatment.  You will be asked to provide written authorization for release of any information.  
 
Private patients (e.g., those patients who are not U of T students or carded athletes) need to obtain a referral from their family physician to book an appointment with one of the sport physicians.  A direct referral for therapy from an external physician is acceptable. Please note that the clinic does not treat patients with workplace or motor vehicles injuries (we reserve the right to cease treatment).

Cancellation Policy
We require 24 hours notice (during business hours) for cancellation without penalty. Our registration form, which must be completed and signed during the first visit to the clinic, details our cancellation policy.
